Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about March 23, 2021, AV was found on the floor “screaming in pain and scared” because he/she had attempted to transfer him/her-self and as a result fell. AV had reddened area to the rib cage. This had been the second time AV had a fall while attempting to self-transfer. The facility failed to properly plan care around AV’s attempt to self-transfer, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-02960 $375.00 fine assessed
