- Findings
- Alleged Victim (AV) relies on the facility for his/her care. AV has a history of falls. AV is non-weightbearing and requires two (2) persons assist for all transfers. On or about March 24, 2021, AV fell and sustained an injury. Facility policy is residents are not to be picked up off the floor without an assessment completed by Med. Tech. Facility video shows AP2 in AV room after reported fall. AV reports AP2 picked AV up off the floor. AP2 denies picking AV up off the floor. AV was transported to hospital with injury. The allegation that AP2 failed to provide basic care and services necessary for AV health and safety was investigated and wrongdoing was unable to be determined. The facility failed to provide necessary care as needed due to transfer and mobility issues and failed to assure a qualified caregiver was present which is a violation of resident’s rights is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i).
