Findings: The facility failed to provide appropriate assessment following an injury fall. On or about February 26, 2021, the Alleged Victim (AV) experienced a fall where it was reported his/her foot “was turned out in an odd manner that would indicate possible fracture”. The failure resulted in AV experiencing a delay in emergency care services, which left AV in pain and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse as defined in OAR 411-020-0002 (1)(b)(A)(ii).
