Findings: The facility failed to implement interventions and appropriately care plan related to Alleged Victims (AV) repeated falls. AV had approximately four (4) falls on or about February 17, 2021, to March 11, 2021. The failure resulted in physical harm in the form of a black eye and fractured finger to AV, which is a violation of residents right, is considered neglect of care and constitutes abuse.
