- The facility failed to follow the Alleged Victim's (AV) care plan to provide two-person transfer assistance with a wheel chair. The failure resulted in AV experiencing a call causing bruising and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
