Findings: The Alleged Victim (AV) has a history of wandering into other residents rooms. Witness #1 (W1) has a history of becoming agitated when others enter his/her room and a history of altercations. Neither AV's service plan nor W1's service plan addresses these issues with interventions. On or about March 21, 2021, AV wandered into W1's room, causing W1 to become agitated. W1 hit AV, causing AV to fall and scrape his/her knee. The facility failed to properly care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
