Findings: The Alleged Victim (AV) has a history of elopement. On or about March 26, 2021, AV was on an outing with Alleged Perpetrator #2 (AP2) and other staff and residents to a local mall. AV was in the group being supervised by AP2. Facility policy is 3 residents to a staff member while on outings. AP2 had 5 residents under his/her care while on this outing. While residents were all having lunch, another resident had an episode, AP2 was assisting that resident when AV got up from his/her table, exited the mall on got onto a city bus. AV was placed at risk for harm. AP2 failed to keep AV safe, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure staff were following policy to ensure resident safety, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
