Findings: The Alleged Victim (AV) has a history of falls, and is care planned to be assisted with ambulation. AV suffered a fall on October 23, 2020 and suffered a bump to the head, AV suffered a second fall October 28, 2020, which resulted in a brain bleed and a cracked pelvis. The facility failed to implement meaningful interventions to ensure AV's safety after the earlier fall. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
