Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about March 30, 2021, AV suffered a fall that was not witnessed. AV was taken to urgent care and was found to have a fracture. AV was moved to a skilled nursing facility for care. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
