Findings: Witness 1 (W1) has a known history of aggressive behaviors. On or about February 19, 2021, W1 wondered into Alleged Victim’s (AV’s) room and when confronted by AV, W1 said it was W1’s room and grabbed AV by the hair and threw AV to the floor. AV had a large handful of hair pulled from AV’s head and landed on AV’ right arm causing soreness to his/her arm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP21-02872 $500.00 fine assessed
