Findings: On or about March 29, 2021, Alleged Victim (AV) hit Witness 1 (W1) with his/her shoe in the hallway of facility. AV has a history of resident-to-resident altercations. The facility failed to protect W1 from AV's aggressive behavior which is a violation of resident rights, is considered neglect of care and constitutes abuse.
