Findings: Alleged Victim (AV) relies on the facility for h/h care. On or about April 01, 2021 AV was to start new eye medication. It was discovered the eye medication was not given and still found sealed. Alleged Perpetrator (AP2) reported he/she had given two (2) of the three (3) types of eye drops and was called away. AP2 planned to come back and administer the last eye drop, but forgot. AP2 signed off that he/she administered all eye drop medication. AP2 failed to administer ordered medication, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system which is a violation or Oregon Administrative rules.
