Findings: On approximately two to four occasions between January 2021 and April 2021, the Alleged Victim's (AV) oxygen levels were not checked by staff as stated in the care plan, leaving AV without oxygen on these occasions. AV suffered dizziness, light-headedness and a fall due to running out of oxygen. The facility's inaction placed AV at risk for serious harm. The facility failed to follow the care plan and provide service to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
