- Alleged Victim (AV) relies on the facility for his/her care. On or about September 15, 2020, AV had begun to show he/she required a higher level of care but there is lack of evidence to show services and care were being provided to AV. The facility failed to properly plan care for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
