Findings: The facility failed to ensure supervision and staff support regarding known behaviors related to the Alleged Victim (AV) and Witness #4 (W4). W4 has had multiple incidents with AV and other residents for months and was service planned to be monitored in the common areas. On April 4, 2021, an incident occurred between the two where W4 hit AV in the head, causing bruising to his/her eye. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
