Findings: On or about April 7, 2021, the Alleged Victim (AV) was found with a soaked brief, which hadn't been changed by the prior shift. AV is prone to skin breakdown and needs his/her brief changed timely to ensure no skin breakdown occurs. AV's skin was very red from not being changed timely. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
