Findings: The facility failed to appropriately care plan regarding Alleged Victim (AV) falls to ensure safety. AV fell twice (2) on or about April 10, 2021, was transported to the hospital for treatment, and found to have fractured wrist. AV had another fall on April 11, 2021 and again on April 12, 2021 and April 14, 2021, which is a violation of resident's rights is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i).
