Findings: On or about April 11, 2021, the Alleged Victim (AV) suffered a fall and sustained a bruise to his/her eye. The AV has had a series of falls in the past and was placed on 4 hour checks. There is no indication from documentation that AV had been checked within the required time. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
