Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV was a known fall risk and had multiple falls in the month of March 2021 and had previously broken his/her hip from a fall. On April 10, 2021, AV was found on the floor from a fall after transferring him/herself to the restroom, was transported to the hospital for treatment and was diagnosed with a hip fracture requiring surgery. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
