Findings: On or about March 25, 2021, Alleged Perpetrator #3 (AP3) gave the Alleged Victim (AV) medication that belonged to another resident. AP3 realized his/her mistake, notified Alleged Perpetrator #2 (AP2) who contacted AV’s Physician. AV did not have any negative consequences to this error, however, AV was placed at risk for harm. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
