Findings: Alleged Victim (AV) relies on the facility for his/her care. The facility failed to provide a safe medication administration system to ensure AVs physician orders were followed for discontinuation of a medication that AV experienced a side effect from. The facility failed to provide a safe medication administration system to ensure AV received needed medication for blood pressure. In both incidents AV experienced unreasonable discomfort which is a violation of resident’s rights, is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i),
