Findings: The Alleged Victim (AV) was a known elopement risk and exit seeker. On or about April 18, 2021, the AV eloped through a patio gate and was found an hour later approximately 2 miles from the facility. The gate likely opened due to a power outage on April 17, 2021 and was not checked by staff to ensure that the gate remained secure after the outage. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
