Findings: The facility failed to provide a safe environment for Alleged Victim (AV). The facility failed to appropriately care plan for Witness 1 (W1) service plan to address her/his history of resident to resident altercation and aggressive behaviors, leading to a resident to resident altercation where W1 slapped AV, causing AV pain. The failure is a violation of resident rights, is neglect of care and constitutes abuse as defined in OAR 411-020-0002 (1)(b)(A)(i).
