Findings: On or about 4/18/2021, the Alleged Victim (AV) left the facility without the knowledge of staff and was found approximately 1 mile from the facility and brought back to the facility by a community member. At that time the facility put interventions in place to reduce the risk of AV leaving the facility alone, however, on or about 5/8/21, AV again exited the facility without staff knowledge, placing AV at risk for harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01724 $188.00 fine assessed
