Findings: On or about April 22, 2021, the facility failed to ensure that Alleged Victim (AV) was receiving the basic care and supervision needed which resulted in AV suffering a fall that resulted in a laceration requiring sutures and a rib contusion. An investigation determined that AV is supposed to be on the "AVAFALL" program with more frequent safety checks per the service plan following on March 13, 2021, but staff reported AV is not on any safety checks, other than once per day. The facility's failure to properly care plan for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03292 $1125.00 fine assessed
