Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about December 13, 2020, AV suffered a fall resulting in an injury requiring hospitalization. While reviewing the documents, APSS discovered AV had a number of falls within a three-month time frame. Between December 13, 2021 through February 18, 2021, AV sustained nine (9) unwitnessed falls. Three falls resulted in injury or complaint of pain. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
