Findings: On or about April 23, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation resulting in W1 bumping AV's head and fracturing AV's wrist. W1 has a history of behaviors with aggression and combativeness. The facility failed to provide a safe environment for AV by failing to protect AV from W1's aggressive behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03026 $1500.00 fine assessed
