Findings: Alleged Victim (AV) relies on the facility for his/her care. AV has a history of falls. The facility failed to appropriately care plan regarding Alleged Victim (AV) falls to ensure safety. AV fell on or about April 20, 2021 and sustained a skin tear to his/her right arm and head. The AV was transported to the Emergency room for treatment. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
