Findings: On or about April 26, 2021, Alleged Victim (AV) had an unwitnessed fall which resulted in a broken hip and cracked pelvis. An investigation determined that the facility failed to ensure appropriate, individualized interventions were being used to mitigate AV's fall risk when he/she was out of his/her bed. The facility's failure to develop or maintain a comprehensive care plan for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
