- Findings
- Alleged Victim (AV) requires 2-person assistance with Hoyer lift transfers. On or about January 12, 2021, Alleged Perpetrator 2 and another staff were transferring AV with a Hoyer lift. AP2 let the Hoyer bar slip and it hit AV under the right eye causing a bruise. AP2’s actions are considered physical abuse. The facility failed to protect AV from physical abuse, which is a violation of resident rights, is neglect of care and constitutes abuse.
