Findings: On or about March 16, 2021, Alleged Victim (AV) was found on his/her bedroom floor after falling and was diagnosed with a fractured right ankle two days later. The facility failed to pursue medical intervention resulting in untreated pain which is a violation of resident rights, is considered neglect of care and constitutes abuse.
