Findings: On or about April 22, 2021, the Alleged Victim (AV) and Witness #1 (W1) engaged in an unwitnessed altercation where W1 pushed AV to the ground, causing a bruise to AV's hip. AV is known to have behaviors that irritate other residents, causing altercations. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
