Findings: Witness #1 (W1) has a history of resident-to-resident altercations as a result of wandering into other residents’ rooms. W1 service plan indicates to keep W1 in common areas to prevent altercations. On or about April 30, 2021, Alleged Victim (AV) reports W1 wandered into his/her room where an altercation occurred. The facility failed to follow care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
