Findings: On or about May 6, 2021, the Alleged Victim (AV) was permitted to ambulate without his/her walker. Staff saw AV without his/her walker and failed to stop him/her or get his/her walker for him/her. AV was a known fall risk and had many falls previously. The action of allowing AV to ambulate without his/her walker placed AV at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02770 $1500.00 fine assessed
