Findings: Alleged Victim (AV) has a known history of falls. Between October 29, 2020, through March 6, 2021, AV has sustained five (5) falls. On or about March 19, 2021, AV had a fall while walking which resulted in a hip fracture. AV was not being assisted by staff prior to the fall. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
