Findings: Witness #1 (W1) is known to go into other residents rooms and recently had a physical altercation with another resident. On or about May 3, 2021, W1 had an altercation with the Alleged Victim (AV), resulting in an injury. The facility failed to have interventions in place following W1's previous incident. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
