Findings: Alleged Victim (AV) relies on the facility to provide a safe environment. Witness 1 (W1) had a known history of resident-to-resident altercations. On or about May 3, 2021, W1 and AV got into an altercation when W1 entered AV’s room. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
