Findings: Alleged Victim (AV) relies on the facility for his/her care. AV sustained four (4) unreported falls, some with injuries. On or about February 17, 2021, AV was found on his/her hands and knees unable to bear weight. On or about March 1, 2021, AV was found on the floor and indicated he/she hit his/her head and was in pain and had a scrape on his/her abdomen. On or about February 19, 2021, AV went into another resident’s room and got into a resident-to-resident altercation resulting in AV being tossed in the ground and smacking his/her cheek on the floor. On or about March 12, 2021, AV was found on the floor with a mouth of “partly coagulated blood”, AV could have been laying there for 30 to 40 minutes before being found by staff. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
