Findings: On or about May 9, 2021, the Alleged Victim (AV) exited the facility through an unsecured kitchen door and was found outside the facility crossing the street. AV was brought back to the facility and was assessed for injuries and none were found, however, AV was placed at risk of harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
