- The facility failed to implement reasonable interventions to address Alleged Victim's (AV) ongoing falls, including falls with injury including a fall that resulted in multiple lacerations, bruising and swelling to AV's face, and nasal fracture. The facility's failure to properly care plan for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
