Findings: On or about May 12, 2021, Alleged Perpetrator 2 (AP2) pushed Alleged Victim (AV) in his/her walker in an unsafe manner that is not approved by manufacture. The walker tipped and AV fell which caused pain in AV's shoulder. AP2's actions is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00180 $188.00 fine assessed
