Findings: Alleged Victim (AV) has a history of falls. Between January 28th through April 27th, 2021, AV sustained twelve (12) falls or had been found on the floor, some with injuries. On or about May 7, 2021, AV suffered a non-injury fall with no complaints of pain and no bruising or swelling. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
