Findings: On or about May 12, 2021, Alleged Victim (AV) suffered a non-injury fall when he/she attempted to self transfer out of his/her bed. An investigation determined that AV had numerous falls within a few weeks time period prior to this incident and that the facility did not have proper preventions/interventions in place to prevent future falls. The facility's failure to properly care plan for AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03426 $375.00 fine assessed
