Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about May 16, 2021, AV had an unwitnessed fall and was discovered by staff on the floor. Staff attended to AV, but failed to contact hospice, the facility nurse, or the family. No medical attention was sought or provided. On May 17, 2021, hospice discovered bruising and notified family members of the fall. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP21-03065 $250.00 fine assessed
