Findings: The facility failed to properly care plan for the Alleged Victim (AV) in regards to his/her known behaviors, placing residents and staff at risk for harm. On May 9th and May 10th, 2021, AV was physically aggressive with staff and residents. Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (AP3) tried without success to keep AV calm, however, were unsuccessful. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-02804 $250.00 fine assessed
