Findings: On or about May 19, 2021, the Alleged Victim (AV) and Witness #1 (W1) were involved in an altercation where W1 hit AV. There were no injuries, however, AV was placed at risk for harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
