Findings: Alleged Victim (AV) relies on the facility for his/her care. AV has a history of falls. AV is care planned to have a pillow under the sheets to prevent falling. On or about May 18, 2021, AV was found on the floor next to the bed. The pillow was not being used. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
