Findings: Alleged Victim (AV) relies on the facility for his/her care. AV moved to the facility on or about April 15, 2021, and at that time, the facility was aware of AV’s wounds. AV’s medical administration records (MAR) do not indicate any topical cream or bandage treatment for wound care. Between April 22nd through May 20, 2021, AV’s wounds increased in number and severity. The facility provided showers to AV on May 16th and 19th, 2021 and did not indicate worsening wounds in documentation or to Home Health. On or about May 20, 2021, AV’s wounds were noticed to be open and severe enough that AV was sent to the hospital. The facility failed to intervene when resident’s condition changed, which is a violation of resident rights, is neglect of care and constitutes abuse.
