Findings: The Alleged Victim (AV) relies on the facility for care. AV’s care plan states facility staff will assist AV with activities of daily living (ADLs) that include washing AV’s body and hair, assist AV with washing their face, brushing their hair and teeth, as well as assisting with dressing each morning and encourage fluid intake. AV was taken to the hospital at approximately 1:45 PM on May 22, 2021, and AV was in a night shirt and depends, had mucus and crust on h/h eyes to the extent that AV could not open them, had a dry mouth with mucus at the corners, and hair stuck in mucus. The facility failed to follow AVs care plan, which caused a loss of personal dignity, is a violation of resident rights, is considered neglect of care and constitutes abuse.
