Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about May 16, 2021, AV was heard yelling from his/her room. When staff entered the room, they found AV on the floor. AV reported he/she was walking with a cup of water and lost his/her balance and fell. AV has experienced three (3) falls in less than 30 days and has sustained injuries in all three falls. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-02841 $500.00 fine assessed
